ISMAA 2009 Registration Form

The 2009 meeting of the ISMAA will be held April 3-4 at Bradley University in Peoria, Illinois. To register, please provide the following information. When you have filled in the form, click on the "Submit Information" button at the bottom of the page. This will take you to a web page where you can confirm the accuracy of the information that you entered. If you have any charges for attending the meeting, you will then be taken to a page where you will be given a choice of sending your payment electronically through Paypal or printing out your registration form and sending a check payable to ISMAA by mail. In both cases your payment will be sent to ISMAA Treasurer Jon Johnson (jonj@elmhurst.edu).
